Academic Support Request Form

Please complete the below form to apply for academic support for a student at your school who is on scholarship with us.

Tutoring Support Process:

Partner School to source and lock in an external tutoring provider
Partner School to complete the below ‘Academic Support Request’ form
MADALAH to approve request and send through a purchase order to the Partner School
Partner School to pay external tutoring provider on invoice and acquit the funds to MADALAH at the end of the academic year
Partner School to re-apply at the end of each term if the student/s require tutoring to continue

Other Support Process:

We can also provide one-on-one support to students via a MADALAH staff member if you think this caters more to their needs. For example, if they are struggling academically and you feel a cultural perspective might be required to support their learning or to help them to navigate their learning, for example, if a student is ESL and needs support switching Aboriginal English to Standard Australian English.
